WA Department of Disabilities- Person Centered Planning (2021- present)
Working with Washington DDA, Gwen’s Guidance is offering Person centered Planning.
Person-Centered Planning is a collaborative, strengths-based approach that focuses on an individual’s goals, preferences, values, and vision for their life despite and societal or physical barriers. Rather than fitting a person into predetermined systems or expectations, the process centers their voice, choices, and unique support needs. As a facilitator, Gwen also honors and brings together any communal, systemic, or personal supports the individual may want to be apart of the process. PCP helps create meaningful, individualized plans that promote autonomy, inclusion, self-determination, and overall quality of life with tangible steps for all parties involved.
